2009 Medical Plan Summaries

ACS offers a variety of medical insurance plans to help you meet the needs of you and your family. Employees and dependents who are enrolled in an ACS medical plan are automatically enrolled in the applicable prescription coverage program at no additional cost.

Employees who are nonexempt and have base annual earnings up to and including $30,000 may enroll in a Limited Benefit Plan. Eligibility for medical coverage is based on your status on your date of hire and will not change if your status or pay change on a later date.

Remember, after one year of service, these employees must choose a new medical option from those described below during annual enrollment, to be effective the following January 1.

For all other eligible employees:

 - Consumer Choice Plan (qualified high deductible health plan)

 - Select Value Preferred Provider Organization (PPO) Plan

 - High Value Preferred Provider Organization (PPO) Plan

 - Health Maintenance Organizations (HMOs) are available in some locations

Your share of the cost for medical coverage is deducted from the first two paychecks of each month on a before-tax basis except for coverage for a domestic partner or a domestic partner’s child(ren), which must be deducted on an after-tax basis.
