Simple Steps to a Healthier Life® Health Assessment is a confidential questionnaire administered by Aetna.  The health assessment is an interactive tool designed to help you understand more about your general health.    Completion of this online health assessment will provide you with a personalized action plan to support you in your efforts to get in shape, eat healthier, lose weight, cope with stress, and more!

If you took the Simple Steps(R) health assessment by December 7, 2009, or within 30 days of your date of hire, you will receive a $10 per month incentive towards your medical plan contributions in 2010, even if you are not enrolled in Aetna medical plan coverage.

The individual results of your Health Assessment will not be shared with ACS.  Your results will not be used to determine your rates or eligibility for medical coverage.  Rather, general information regarding the overall health of employees will be used to help improve our ongoing wellness initiatives.
