What is a Health Management Program?
The ACS Health Management Program helps employees who have chronic conditions better manage their health by actively working with the employee and the employee’s doctor(s) to help improve care. Employee’s health information is always kept confidential and will not be shared with ACS. The Health Management Program is a resource setup to help you and your doctor make informed decisions about your health. The program does not take the place of your doctor; instead, it’s a proactive way to improve your health and wellness.

The disease management program isn’t new. In fact, ACS has had Aetna’s disease management program in place since 2007. The name “Health Management” was chosen for ACS’ disease management program because the title “disease management” is not reflective of ACS’ intention for the program. In fact, the name “disease management” can be very misleading because when individuals hear the word “disease” they often think of conditions like cancer, HIV or Parkinson’s disease. However, the truth is that the disease management program covers a much broader array of conditions that people often do not think of as diseases. Examples of these conditions are diabetes, migraines, high cholesterol and arthritis. These conditions are typically chronic or recurring frequently and need to be managed. Research has shown that better management of these conditions has dramatic improvement on a person’s health.

What Conditions are Covered in Health Management?
There is a wide variety of medical conditions covered by the Health Management Program. You probably know someone who has one of these conditions or you may have one yourself. As you read this list, make a mental note of any conditions you and your doctor have talked about previously. The following are examples of conditions that may be covered by your applicable ACS Health Management program.

Heart & Blood Vessel Conditions

  • Blood clots
  • Disease of leg arteries/PAD
  • Heart attack and angina
  • Heart failure
  • High blood pressure*
  • High cholesterol
  • Stroke

Weight Management

  • Overweight/obesity*

Bone and Joint Conditions

  • Arthritis
  • Chronic back pain
  • Osteoporosis

  • COPD (Chronic Obstructive Pulmonary Disease)

Neurologic Conditions

  • Depression
  • Migraines
  • Parkinson’s Disease

Stomach & Intestine Conditions

  • Chronic hepatitis B or C
  • Crohn’s Disease
  • GERD/gastric reflux disease
  • Stomach ulcers
  • Ulcerative colitis

Kidney Conditions

How Do I know if I am Eligible to Participate?
The Health Management Program reviews information from member’s medical and pharmacy claims and other sources to determine if a member has a chronic condition that would benefit from the Health Management program. Typically, members who are identified as eligible for program participation if they have at least one covered conditions.
The Health Management Program will reach out to you via telephone calls and letters. The first call from the Health Management Program may be a pre-recorded message explaining how to join the program; be sure to listen to the entire message and follow the instructions carefully.

Can my Eligible Dependents Participate in the Program?
If your spouse/domestic partner or children (over the age of 18) are enrolled in a qualified ACS medical plan, they are eligible to participate in the ACS Health Management Program if they have a covered condition. The ACS Health Management Program has special pediatric programs for dependents under the age of 18. Call ActiveHealth at 1-800-662-7092, Monday-Friday, 8:30 am to 9pm (ET) to speak with a representative about whether your dependents are eligible.
